Name of an agency in Dallas, Texas area that teaches Special Incident Report writing skills to non-profits?

I attended a training in Sacramento, Ca. a few years ago. This training went over the specifics of Special Incident Report Writing. I worked for a private, non-profit that worked with individuals with developmental disabilities.This company was from the Dallas, TX. area but I cannot remember the name of the company. The training was quite good.
